Du schriebst:

   device-mapper major = 252
   Fatal: raid_setup: stat("/dev/disk/by-id/ata-QEMU_HARDDISK_QM00005")

Hier sehe ich nur folgenden Unterschied:
Einmal major 252 und einmal major 253.
